The Healthy Living Center of Excellence represents a collaboration of community-based organizations, aging service providers, healthcare systems, governmental agencies, and healthcare payors; all with the shared goal of transforming the traditional healthcare delivery system. We empower older adults to partner with traditional medical providers and community based social service agencies to achieve better health and better health care at lower costs. These programs all occur in community settings and are designed to empower adults living with one or more chronic conditions to take a more active role in their health care. CHRONIC DISEASE SELF-MANAGEMENT PROGRAM-this program is designed for adults and their caregivers living with daily challenges of one or more ongoing health conditions, participants will learn different methods for managing pain, fatigue, and other symptoms. Better Choices, Better Health is the online version of this program.TOMANDO CONTROL DE SU SALUD-this program is the culturally appropriate Spanish version of the Chronic Disease Self-Management Program.DIABETES SELF-MANAGEMENT PROGRAMS-this 6 week program is for adults living with diabetes and their caregivers to learn several techniques for monitoring diabetic conditions. LIVING LA VIDA DULCE-this program is the culturally appropriate Spanish version of Diabetes Self-Management.CHRONIC PAIN SELF-MANAGEMENT PROGRAM-this 6 week program teaches adults suffering from chronic pain simple techniques to better manage their pain, improve sleep, increase energy, eat healthier, and more.MATTER OF BALANCE-this program encourages participants to see falls as controllable through increased activity and awareness of fall hazards. The 8 week sessions include exercises to improve balance, flexibility, and overall strength.THE SAVVY CAREGIVER PROGRAM-is for caregivers actively caring for a friend or family member living with Alzheimer's Disease or related dementia in the community. The goal of this informative and interactive training is to increase caregiver knowledge, skills, self-efficacy, and well-being.HEALTHY EATING - this program is for older adults looking to improve nutrition and increase physical activity, the program promotes heart and bone health and aids in the prevention/management of chronic health conditions.HEALTHY IDEAS-this is a community depression program designed to detect and reduce the severity of depressive symptoms in older adults with functional limitations. This program incorporates 4 components into the ongoing service delivery of social service programs serving older individuals in their homes.ENHANCE WELLNESS-this individualized, community-based wellness intervention is designed to promote the health and functioning of older adults at risk of decline. This program helps identify potentially modifiable risk factors for disability and promotes behavior change to reverse risk factors with a goal of maintaining or increasing functional status in the community.CANCER: THRIVING AND SURVIVING PROGRAM-this program is designed for adults facing the daily challenges of cancer treatment or how to cope with end of treatment. POSITIVE SELF-MANAGEMENT PROGRAM FOR HIV-this program is designed to enhance regular treatment and HIV-specific education for those living with HIV.
