Since the inception of the Young Adult Community for Crohn's and Colitis (YACCC), in May of 2021, we have been striving to inform, engage, and empower young adults living with Inflammatory Bowel Disease (IBD) by supporting them in their transition from pediatric to adult IBD care and beyond. Through our collective lived experiences, we have come to understand that the transition to adulthood with a chronic illness can be a difficult and isolating experience. Recognizing the gaps in our own care inspired us to create YACCC as a means of improving the transition experience for other young adults living with IBD through education events, social engagement, and advocacy.
